The Following Forms Are Information About Each Meet

All the forms you need for the Cal Poly, Fortuna and Championship Meets are located on this page. You must have the liability forms for Cal Poly and Championships completed and signed by parents to run in the meets. The schedules, payment forms and other information are also on this site. Cal Poly information is in a white background, Fortuna is blue and the Campionships are in black.

Payment forms are due for Fortuna and the Championships, but Cal Poly does not require payment.
